Signing up for Bally's Membership?

If I were to signup for a Bally's membership under a friends' plan, can anyone give me an estimate of the monthly rates? Also, if I use my dad's credit card to pay, would my dad have to be present because a 2 wks ago, someone from Bally's said no and someone I just called now said yes.
